Fall Retreat: Medical Release and Permission Form

October 11-13, 2019 | Hey Youth and Parents!
The summer is still warm and there is plenty to do but guess what. The fall is almost here! Fall means Fall Retreat! There are only 9 weeks left until our first annual Fall Retreat at East Iowa Bible Camp.

Who:
All students 6th grade – 12th grade (during the 2019-2020 school year) (Both Thrive & Rooted). We will be looking at having at least 20 students and adult leaders

What:
Retreats are a special time for youth groups. It is when students get to “escape” from their normal routine of school and life and go to spend time together and to encounter God. Through a series of teachings, intimate worship, extended fellowship and activities, devotionals and God-time, and good food, students will encounter God and move closer in their relationship with God. This fall, we will be focusing on the Shema and what it looks like to be an actual whole-hearted follower of Jesus.

When:
Friday, October 11th (4:30pm) to Sunday, October 13 (9am)

Cost:
$125 per student.

I have multiple students. Is there a discount?
Yes! $125 for the first student, $115 for the second, and $100 for the third.

What’s included in the cost?
Food & Lodging:
East Iowa Bible Camp lodging and exclusive use to their facilities, plus 5 meals (Friday night dinner, Saturday Breakfast, Lunch, Dinner, Sunday Breakfast).
Transportation:
We will be using 15 passenger vans and a U-Haul trailer to transport to and from East Iowa Bible Camp.
Sweet Sweatshirt:
Each student will receive a sweet sweatshirt that they will want to continue to wear.

What will my student be doing while away?
Activities: Basketball in the barn, dodge ball, octaball, steal the bacon, carpet ball, table games, board games, disc golf, miniature golf, soccer, fishing, boats, campfires.
Sessions: 4 sessions with teaching and worship.

What should my student bring?
Sheets & blankets or sleeping bag, pillow, bath towels, laundry bag, outdoor & indoor shoes. Bible. Pen. Toiletries. Flashlight. Clothes. Raincoat or poncho. Bugspray. Sunscreen. Food/Candy to share in common area. Board Games to share. Shower Shoes.

What should my student NOT bring?
Food/Candy packed away and kept in cabin (attracts critters). Electronics (video games, iPods, cell phones, etc.). Valuables of any sort. Weapons. Fireworks. Tobacco Products. Vape Pens. Pets.

This looks like a good time. Can I go as a leader?

For your information, we expect each student to conform to these rules of conduct


No fighting, fireworks, or explosives
No offensive or immodest clothing
Participation with the group is expected
Respect property
Respect one another, staff, and adult leaders
Respect and comply with event schedules

Students who fail to comply with these expectations may be sent home at their parents’ expense.

I, the student, have read the rules of conduct, the above evaluation of my health, and permission to participate in Oak Grove Churchs youth activities. I agree to abide by the stated personal limitations and code of conduct.

 

This consent form gives permission to seek whatever medical attention is deemed necessary, and releases the Church and its staff of any liability against personal losses of named child.

I/We the undersigned have legal custody of the student named above, a minor, and have given our consent for him/her to attend events being organized by the Church. I/We understand that there are inherent risks involved in any ministry event, and I/we hereby release the Church, its pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my/our childs involvement. In the event that he/she is injured and requires the attention of a doctor, I/we consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the Church, I/we agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I/We also acknowledge that we will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider. I/we also agree to bring my/our child home at my/our own expense should they become ill or if deemed necessary by the Youth Leaders. 



Photos will be taken for the use of promotion of Youth Ministry at Oak Grove Church via social media, church website, and print media.
